  • Primary Care Networks | RHA PCA

    What are Primary Care Networks Primary care networks are based on GP-registered lists, typically serving local communities of around 30,000 to 50,000. As primary care networks develop they will ultimately comprise of multidisciplinary teams such as physiotherapists, social prescribers, pharmacists, paramedics and community nurses. The Networks should be small enough to provide personalised care, but large enough to benefit from economies of scale, through better collaboration between GP practices and community care providers.   • Primary Care Academy

    < Back Primary Care Academy The Primary Care Academy is provided by Rochdale Health Alliance and works in collaboration with NHS Heywood, Middleton and Rochdale Clinical Commissioning Group. The Primary Care Academy was launched in September 2018 to focus on training, education, workforce development and research across the entire primary care workforce in HMR. It aims to create and provide new opportunities for people working in primary care and entering primary care, to shape, inform and influence the career opportunities and environment they would like to work in. We will collaborate with, NHS Heywood, Middleton and Rochdale Clinical Commissioning Group, Higher Education Institutes and other identified stakeholders in supporting service redesign, education and training of the workforce across primary care. The HMR Primary Care Academy is supporting the implementation of the NHS strategic direction, the Five Year Forward View and the programmes that underpin it, particularly the GP workforce 10 point plan, which stresses the urgent need to recruit doctors and nurses into general practice, retain current staff and support returners. Our Key priorities are: To help future proof General Practice in HMR – providing clinical and non-clinical staff with the necessary workforce skills, values and support tools, such as mentoring to improve the quality and continuity of safe primary care services in HMR To promote HMR as the first-choice career location – influence GPs, Nurses and other health professionals planning their future and highlight the many benefits of living and working in the area with an emphasis on introducing roles emerging and apprenticeships available Valuing Primary Care Staff – their role is essential in securing the sustainability of the NHS and acknowledges the need to nurture and support career development. Recruitment and retention- initiatives to support GP Practices Grow our own staff – working with local training providers, schools, colleges and universities Support practices and system leaders – respond to the changing needs of local health and care systems and helps to avoid future vulnerability by understanding and planning for future change now Research and development – to identify those members who would like to be involved in research and form links with universities/networks.     Rochdale Health Alliance Rochdale Health Alliance (RHA) was established in 2016, by GP practices from across the Rochdale Borough. Our Vision: To work collaboratively with members, partners, and stakeholders to improve care and health outcomes for patients To sustain the future of local Primary Care Practices To be representative and supportive of all practices and lead the design and delivery of new ways of working - Ensuring local GP services are at the heart of the integrated system. We have 31 member practices across Heywood, Middleton and Rochdale and have a dedicated tea m on hand to support practice needs. Our dedicated team has expertise that can provide support and expertise in finance, HR, communications, project management and strategy, and our aim is to ensure we share our knowledge to help our members improve their resources. RHA currently supports 6 Primary Care Networks with back-office support such as finance, recruitment, contracts, employment of ARRS staff, training and development as well as providing services such as the mass COVID vaccination programme for patients throughout 2021-2022. Our Network Support Managers and Officers work collaboratively with each of our PCNs to understand their individual needs and create a bespoke level of support for them all. Heywood, Middlet on and Rochdale Primary Care Academy is an integral part of RHA and develops the primary care workforce, in order to facilitate the recruitment and retention of well-trained and motivated staff. RHA currently provides the following services which provide additional access to general practi ce appointments: In Hours Same Day GP Appointments Service. GP Practices are under extreme pressure and appointments can be difficult to get due to this. To try and support patients who after a triage at their own practice, can be booked into one of these appointments so that they are seen on the same day if clinically required . Enhanced Access RHA is commissioned by 5 Primary Care Networks to provide GP and other healthcare routine appointments outside of Practices core Hours. These appointments are generally provided between 18:30 pm and 20:00 pm and Saturdays between 9 am and 17:00 pm Sunday and Bank Holiday Service RHA is commissioned to provide a service on Sundays and bank Holidays to provide GP and other healthcare appointments on these days between 08:00 and 16:00 on Sundays and Bank holidays. Many changes, both national and local, have happened over the past 6 years and throughout all those changes RHA has adapted to ensure that the organisation is able to continue to grow and deliver on our commitments .     < Back Alex Fagan Access Administrator My name is Alex 😊 I started working for RHA in November 2019 as an administrator. This was my first role in health care and grew into the Business Support Officer for the Heywood PCN. My administrator role developed quickly with COVID 19, becoming the lead administrator for the housebound and care home vaccinations. With the support of all the team my skills developed, and I gained more confidence within my work and was inspired to join the HART team. For a few months I worked alongside 2 nurses and an HCA learning about the needs of the transient and vulnerable community. With the support of the Primary Care Academy, I was able to achieve my Level 1 in Functional Skills Maths. A subject I have never been strong at, but RHA and the Academy really supported me through my learning. I worked closely with the Heywood Clinical Director (Dr Z Mohammed) and the PCN Manager (Adele Hardacre) as a Business Support Officer, supporting with the daily tasks of working within a PCN. I am a trained Phlebotomist and provided the Network with domiciliary Phlebotomy, workd with the ARRS staff team as a first point of contact for any queries and arrange events for the PCN in which they educate the public on the importance of their health needs and what the PCN team can do. I am now the Senior Administrator for the Access Service, which is additional primary care appointments out of hours (evenings and weekends) within HMR. I support the Service Manager with any tasks relating to the service, work on the rota of bank staff who work at the hubs. With the support of HR onboard new staff to the bank, create and ensure new staff receive an induction and information on the service is kept up to date (including email communications and updates to the RHA Website). I am currently awaiting a start date in BTEC Diploma in Team Leading Level 2 to develop my skills. Working in Primary Care is so interesting and always presents a new challenge as every day is different. When I started with RHA I had no idea how supportive and caring the team would be and love how they encourage you to grow and develop into a happy and professional career. When I’m not working, I enjoy doing my own nails (which I learnt in lockdown) binge watching Netflix seasons my fave at the moment is The Crown and trying new restaurants…and cocktails 😉.
