Dr Ghafoor always goes the extra mile with his patients. He always tries to follow up clinically difficult consultations. He 'gets back to people' when he says he will. His patients always leave feeling like they have been listened too. The HART team Elaine and Rachel have built positive relationships across HMR and are a point of contact for those often in crisis to support with their health and wellbeing needs. They work across a range of hostels, supported accommodation, outreach centers, breakfast clubs and drop ins
They look for different ways to improve the service and reach differnt groups
They deliver care with kindness, compassion ,dignity , laughter and sometimes tears
There's is not an easy job supporting people and often the results can take months if not years to see improvements in health and wellbeing as other areas are addressed

Dr Aziz works so hard at the practice he goes above and beyond for the patients at the practice and really helps the admin team out by promptly completing and admin tasks sent to him
